Publicly subsidized Long-Term Care homes that provide 24-hour professional care and supervision to adults in a supportive and secure environment.
Publicly subsidized Long-Term Care Services are available to adults with complex medical and cognitive care needs, with an assessed and urgent need for 24-hour care. Services are aimed at meeting the complex health needs of those admitted to a care home, and include:
  • Accommodation
  • 24-hour nursing care
  • Other professional services such as nursing, physical therapy, social work and nutritional support
  • Hospitality services (meals, housekeeping, recreational activity programs, emergency response)
  • Personal care assistance
  • End-of-life care and information.

Eligibility and Access Information
  • Placement in a publicly subsidized long-term care facility is based on the urgency of the individual’s current care needs, not anticipated decline. Only clients who meet eligibility criteria will be placed on a waitlist.
  • Patient may be eligible if they meet Home and Community Care citizenship and provincial residency requirements, and admission criteria for one of Island Health's long-term care homes.
  • An Island Health Case Manager will determine eligibility by conducting a thorough, standardized assessment of need and urgency for long-term care.
  • The results of the assessment may be to provide a range of services in the home and community to enhance care and support that the patient is already receiving. The goal is to remain in the comfort of the home as long as the patient is safe, and their care needs can be met.
  • If there is a hospital visit/stay, the hospital care team there will work with the Case Manager to assist the patient in the wait for placement in long-term care, if applicable.
  • When the time comes to move to long-term care, the Case Manager will guide the patient and family through the process to ensure everyone knows what to expect at each stage.
